Revolutionizing Diagnostic Accuracy

Doctors acknowledge the potential benefits of AI assistance in their work. Dr. Adam Rodman conducted a study to explore these advantages, anticipating that his colleagues using ChatGPT-4 would achieve better diagnostic outcomes than those relying solely on their judgment. However, the findings were unexpected and left him astonished.

The study involved 50 doctors from various American hospitals, including both interns and residents, who were tasked with diagnosing six real clinical cases, each drawn from a pool of 105 cases compiled since the 1990s. These cases had never been previously published, ensuring that neither the participants nor ChatGPT had prior knowledge of them. Only one case was shared ahead of time, complete with examples of both correct and incorrect responses to illustrate expectations.

The participants were split into two groups: one utilized ChatGPT for diagnosis, while the other proceeded without AI assistance. The results were revealing. The control group, operating without AI, achieved an average score of 74%, while those using the chatbot scored slightly higher at 76%. In stark contrast, ChatGPT alone achieved an impressive average of 90%. What accounts for this disparity?

In the aftermath of these findings, Dr. Rodman sought to decode the results. He identified two primary factors. Firstly, doctors often exhibit skepticism towards AI recommendations, especially when they diverge from their own assessments. Conversely, the presence of ChatGPT proposing alternative hypotheses can inadvertently reinforce their own, which may not always be accurate, as evidenced by the study’s outcomes.

This observation aligns with insights from Laura Zwaan, a researcher focused on clinical reasoning and diagnostic errors: “People are generally too confident when they believe they are correct.” The second contributing factor to the study’s results is more straightforward: many doctors struggle to maximize the capabilities of ChatGPT.

Dr. Jonathan H. Chen, a co-author of the study, noted that most practitioners treated AI like a basic search engine, posing questions such as, “Is cirrhosis a risk factor for cancer?” Ultimately, “only a small number of doctors recognized that they could copy and paste the entire case into the chatbot and request a comprehensive response.”
