Canada stunned the Czechs with three goals within three minutes and 19 seconds in the second half at Nokia Arena.
Kent Johnson hit the target between the nets on the powerplay of Adam Lowry and Matthew Barzal.
David Krejci opened the scoring for the Czech Republic, but Cozens tied the game late in the first half. Cole Sillinger completed the mark.
The representatives of the maple leaf, defending champions of the tournament, will face Finland in the final on Sunday.
Finland edged USA 4-3 in the other semifinal earlier in the day. The Americans will meet the Czech Republic in the small final for bronze.
